Greater Insurance Service offers a TransChoice® Health Indemnity Insurance underwritten by Transamerica Life Insurance Company, Home Office, Cedar Rapids, IA.  You have the ability to choose between two limited medical indemnity plans, which are designed to pay indemnity benefits for basic health care   services.  Administration of the TransChoice® policy is provided by Key Benefit Administrators, Inc.  Policy form series CP200100/CC200100.
