Hospital Indemnity Plan

This policy is issued on a simplified issue basis.  Benefits available to members ages 16-69, benefits available for most industries, plans are not age or gender rated.  Members must be actively at work at least 20 hours per week to be eligible.

Benefits Explanations

Hospital Confinement

¨ Pays the Daily Benefit selected for Hospital Confinement as a resident bed patient due to covered Sickness or Injury. 

First Hospital Confinement

¨ Pays a Lump Sum Benefit for the Insured’s First Hospital Confinement for covered Sickness or Injury during the  Calendar Year.

¨ The benefit amount depends on the length of Hospital Confinement:

Specified Injury

¨ Pays a specific amount for an Injury as indicated within the Rider for: Appliances; Ambulance; Blood/Plasma; Burns; Dislocations; Eye Injuries; Fractures; Ruptured Disk; Tendons; Torn Knee Cartilage; and Gunshot Wound. 

   (Not available in Connecticut or Kansas)

Surgical Plus Benefits

¨ Pays the % listed in the Surgical Schedule times the maximum Surgical Benefit shown in the Policy Schedule, for listed surgery performed by a Physician in a Hospital, Ambulatory Surgical Center or Physician’s office for  covered Sickness or Injury.  (If more than one surgical procedure is performed  during one operative session, the Rider will pay one benefit for the greatest surgical procedure.)

  Accidental Death & Dismemberment

¨ Provides a death benefit in additional to all other policy benefits if an insured’s death occurs within 90 days of a covered injury and was a result of a covered injury.  Also provides a lump sum payment for dismemberment or irrevocable loss of sight resulting from a covered injury within 90 days of a covered injury
