Disability InsuranceThis policy is issued on a simplified issue basis. Benefits available to members ages 16-69, benefits available for most industries, plans are not age or gender rated. Members must be actively at work at least 20 hours per week to be eligible. Sample Plan Options ¨ Plan Description - Pays a monthly benefit for total disability resulting from a covered injury or sickness. Benefits begin at the end of the elimination period and continue while totally disabled until the purchased accident benefit period expires. Accident Benefit not payable if Workers Compensation or similar law pays. ¨¨ Accident Elimination Period ¨ 0 Days ¨ Sickness Elimination Period ¨ 14 Days ¨ Benefit Periods ¨ 6 Months ¨ 1 Year ¨ Minimum Issue ¨ $300 ¨ Maximum Issue ¨ $5,000
*Monthly benefit may not exceed 60% of income
